Job Summary

We are seeking a Nurse Practitioner or Physician Assistant to join our Family Medicine team in Chandler, TX. This role offers the opportunity to provide comprehensive, patient-centered primary care in a collaborative outpatient setting. The ideal candidate will be committed to building long-term patient relationships and delivering high-quality care to patients across all ages.

Key Responsibilities

  • Provide full-spectrum primary care including preventive services, acute care, and chronic disease management
  • Perform patient evaluations, wellness exams, and routine screenings
  • Order and interpret labs, imaging, and diagnostic studies
  • Develop and manage individualized treatment plans in collaboration with supervising physicians
  • Educate patients on disease prevention, medications, and lifestyle modifications
  • Maintain accurate and timely documentation within the EMR
  • Collaborate with physicians and clinical staff to ensure coordinated patient care

Qualifications

  • Current Texas license as a Nurse Practitioner or Physician Assistant
  • National certification (AANP, ANCC, or NCCPA)
  • Previous family medicine or primary care experience preferred
  • Strong clinical judgment and communication skills
  • Ability to work independently and as part of a collaborative team
