Job Summary
We are seeking a Nurse Practitioner or Physician Assistant to join our Family Medicine team in Chandler, TX. This role offers the opportunity to provide comprehensive, patient-centered primary care in a collaborative outpatient setting. The ideal candidate will be committed to building long-term patient relationships and delivering high-quality care to patients across all ages.
Key Responsibilities
Provide full-spectrum primary care including preventive services, acute care, and chronic disease management
Perform patient evaluations, wellness exams, and routine screenings
Order and interpret labs, imaging, and diagnostic studies
Develop and manage individualized treatment plans in collaboration with supervising physicians
Educate patients on disease prevention, medications, and lifestyle modifications
Maintain accurate and timely documentation within the EMR
Collaborate with physicians and clinical staff to ensure coordinated patient care
Qualifications
Current Texas license as a Nurse Practitioner or Physician Assistant
National certification (AANP, ANCC, or NCCPA)
Previous family medicine or primary care experience preferred
Strong clinical judgment and communication skills
Ability to work independently and as part of a collaborative team
Schedule & Compensation
Full-time outpatient schedule
