About the Role
Remote LPN/LVN – Nevada
Work Arrangement: Fully Remote (Nevada LPN/LVN license required) Schedule: Flexible | Part-Time or Full-Time Licensure: Active Nevada LPN or LVN (Required)
Role Overview: PharmD Live seeks a proactive Licensed Practical Nurse (LPN) or Licensed VocationalNurse (LVN) to join our remote care team. This position supports patients enrolled in chronic care management (CCM) and remote patient monitoring (RPM) programs. The ideal candidate will provide high-quality patient care, education, and coordination, ensuring adherence to care plans while collaborating with multidisciplinary teams.
Key Responsibilities:
Conduct virtual outreach with patients to review care plans and provide education
Monitor patient health status via RPM tools and escalate clinical concerns appropriately
Document all interactions in compliance with CMS, payer, and internal quality standards
Coordinate post-discharge follow-up and specialist appointments
Collaborate with nurses, pharmacists, and other care team members to optimize outcomes
Foster patient engagement, adherence, and trust through empathetic communication
Required Credentials:
Active Nevada LPN or LVN license in good standing
Minimum of 2 years of clinical nursing experience (telehealth or chronic care preferred)
Strong verbal and written communication skills in English
Competent in patient documentation and clinical judgment
Experience with CCM and/or RPM workflows is a plus
Ability to work independently in a HIPAA-compliant, remote environment
