Case Manager LHH is partnering with a mission-driven nonprofit organization in El Cajon to hire a Case Manager for a contract-to-hire opportunity. This role is ideal for someone who is compassionate, organized, and experienced in supporting individuals through coordinated care and services. You'll play a key role in providing direct client support, connecting individuals to critical resources, and contributing to meaningful outcomes within a collaborative and purpose-driven environment.
Responsibilities: • Provide direct case management services to clients, including assessments, service planning, and ongoing support • Connect clients with community resources such as housing, healthcare, employment, and social services • Monitor client progress and maintain accurate, timely case notes and documentation • Advocate for clients and assist in navigating complex systems and programs • Maintain regular contact with clients to ensure continuity of care and goal progression • Collaborate with internal teams and external partners to coordinate services • Support crisis intervention efforts and respond to urgent client needs as they arise • Ensure compliance with program guidelines, confidentiality standards, and reporting requirements
Qualifications: • 1+ years of experience in case management, social services, or a related field • Strong knowledge of community resources and support services • Excellent interpersonal and communication skills • Highly organized with strong documentation and time management abilities • Ability to manage a caseload and prioritize effectively in a fast-paced environment • Comfortable handling sensitive and confidential information with professionalism • Proficient in Microsoft Office; experience with case management systems is a plus
Compensation & Schedule: • $20-$24/hour • Contract-To-Hire opportunity • Full-time, Monday-Friday (schedule may vary based on program needs) • Fully onsite in El Cajon If you meet the qualifications, please apply today!
