About the Role
Amedisys is seeking a Medical Social Worker to join our PRN Day shifts in Oklahoma City, delivering home health services to patients in need. You will assess social, financial and emotional needs and coordinate care with the health care team to support patients and families at home.
Responsibilities include providing social work services per the plan of care, educating families, and participating in discharge planning, with access to telehealth, paid time off and a 401(k) plan where eligible.
